Notice of Privacy Practices
Applied Innovative Psychiatry · Aspera, LLC
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Effective date: October 24, 2026
Our commitment to your privacy
Applied Innovative Psychiatry, a DBA of Steven Mee, M.D., a Medical Corporation, together with Aspera, LLC, at 3662 Katella Ave, Suite 206, Los Alamitos, CA 90720, protects the privacy of your health information. This notice explains how we may use and share your protected health information ("health information"), your rights, and our legal duties.
We are required by law to:
- Keep your health information private and secure
- Give you this notice of our legal duties and privacy practices
- Follow the terms of the notice currently in effect
- Notify you promptly if a breach occurs that may have compromised the privacy or security of your information
As a California provider, we also follow the California Confidentiality of Medical Information Act and other state laws. Where state law gives your information greater protection than federal law, we follow the stricter standard.
How we use and share your information
We may use and share your health information without your written authorization for these purposes:
- Treatment. To provide, coordinate and manage your care, including with other clinicians involved in your treatment. For example, with your consent, we may coordinate with a therapist who referred you to us.
- Payment. To bill and receive payment for your care. Because we do not contract with insurance plans, this usually means billing you directly and preparing superbills you can submit to your insurer. If you finance care through a third party such as CareCredit, we share only what is needed to process payment.
- Health care operations. To run our practice and improve care, such as quality review, staff training, licensing and accreditation, and legal and financial services.
- Appointment reminders and health information. To contact you about appointments, and about treatment options or health services that may interest you.
Other uses and disclosures permitted or required by law
We may also share your health information without your authorization in these situations, as the law allows or requires:
- As required by law, including to the U.S. Department of Health and Human Services to show we comply with federal privacy law
- To prevent a serious threat to your health or safety, or to the health or safety of others, including a duty to warn an identified potential victim under California law
- Public health and safety, such as reporting suspected abuse or neglect of a child, elder or dependent adult, preventing disease, and reporting adverse reactions to medications
- Health oversight, such as audits, inspections and licensing activities
- Lawsuits and legal proceedings, in response to a court order, or a subpoena when the law's requirements are met
- Law enforcement, in limited circumstances the law permits
- Coroners, medical examiners and funeral directors, as needed for their duties
- Organ and tissue donation requests
- Research, when approved through a process that protects your privacy
- Workers' compensation claims, as the law allows
- Specialized government functions, such as military, national security and protective services
Uses that need your written authorization
For any purpose not described in this notice, we will ask for your written authorization first. In particular, we will not do the following without it:
- Psychotherapy notes. Most uses and disclosures of a therapist's separate notes from therapy sessions
- Marketing. Using or sharing your information for marketing purposes
- Sale of information. We never sell your health information
You may revoke an authorization at any time in writing, except where we have already acted on it.
Your choices
You can tell us your preferences in these situations:
- Family and friends. Whether we may share information with family members, close friends or others involved in your care or payment. If you cannot tell us, for example in an emergency, we may share information when we believe it is in your best interest.
- Disaster relief. Whether we may share information to help notify family of your location and condition.
- Fundraising. We do not currently fundraise. If we ever contact you for fundraising, you can ask us not to contact you again.
Your rights
To use any of these rights, contact our Privacy Officer (below). Some requests must be in writing.
- See and get a copy of your records. You can ask to see or get an electronic or paper copy of your health information. We will respond as the law requires and may charge a reasonable, cost-based fee.
- Ask us to correct your records. If you think information is incorrect or incomplete, you can ask us to amend it. If we say no, we will explain why in writing.
- Ask for confidential communications. You can ask us to contact you in a particular way, such as a different phone number or address. We will agree to reasonable requests.
- Ask us to limit what we use or share. We are not required to agree, except that if you pay for a service in full yourself, you can ask us not to share information about it with your health plan, and we will agree unless the law requires us to share it.
- Get a list of disclosures. You can ask for a list of times we shared your health information in the six years before your request, with certain exceptions such as treatment, payment and operations.
- Get a copy of this notice. You can ask for a paper copy at any time, even if you agreed to receive it electronically.
- Choose someone to act for you. A medical power of attorney or legal guardian can exercise your rights. For patients under 18, California law gives minors control over certain records, and parents' access may be limited in some situations.
- File a complaint. See below.
